Macronutrients Consult

Once I receive your completed personal intake form, I will email you the calculated macronutrients and a basic outline on how to proceed. 

All services will begin with me emailing you an intake sheet.  Once that is complete and returned to me, we will schedule your telephone consultation.

*Note:  I require at least 24 hours of notice for telephone appointment cancellations.  If you miss a scheduled phone call without sufficient notice, you will forfeit the time.
